The Ministry of Health and Social Services is investigating the deaths of a mother and her newborn baby at the Lüderitz District Hospital on 27 October 2025, with Health and Social Services Minister Esperance Luvindao saying it is too early to determine whether medical negligence occurred.

Luvindao made the remarks in Parliament last week while responding to questions from Independent Patriots for Change (IPC) Member of Parliament Lilani Brinkman regarding the incident.

The minister expressed condolences to the bereaved family and pledged accountability.

According to Luvindao, the incident was reported within 24 hours, an initial investigation was completed on 01 November 2025, and a multidisciplinary team reviewed the case on 02 November.

A hospital committee submitted its report on 06 November, while a regional mortality meeting was held on 10 November. Investigations at the national level are ongoing, she said.

Preliminary medical reports indicate that the patient was monitored and that an emergency Caesarean section was deemed necessary following foetal distress and subsequent haemorrhage.

However, Luvindao said final clinical reviews must be completed before conclusions can be drawn.

“It would be premature and procedurally incorrect to make a definitive pronouncement on whether medical negligence occurred in this specific case,” she said.

The ministry said suspected negligence could result in administrative investigations, retraining, supervision, restrictions on clinical privileges or referral to the Health Professions Councils of Namibia.

Meanwhile, Luvindao said the ministry has strengthened emergency maternity care through updated clinical protocols, improved medical documentation, additional nursing support and the availability of a specialist obstetrician.

Investments have also been made in medical equipment, round-the-clock oxygen availability and blood transfusion capabilities.

Training and emergency simulations are continuing, while further corrective measures will depend on the findings of the investigation.

The minister further said bereaved families could access free, confidential support from hospital social workers, counsellors and spiritual care providers.
In this case, the family was consulted and consented to the transfer of the deceased to Windhoek for an autopsy.
